You want a real checkup, not a 12-minute physical.
You also don't want the incidentaloma factory.

Somewhere around 40 or 50, the annual physical starts to feel too thin — and the alternative on offer is a $2,500 "executive screening" that scans everything, finds harmless spots on half its customers, and launches them into months of follow-up imaging for things that were never going to hurt them. The Sane Physical is the third option: evidence-based depth — the tests that have actually earned their place for someone your age with your history, a real physician hour to go through everything — and, in writing, the list of what we recommend you NOT scan, and why.

Not for emergencies — and not for symptoms. If you have chest pain, severe shortness of breath, or fainting right now, call 911. A yearly deep-dive is for the asymptomatic; new symptoms belong with your own doctor now, not on next month's schedule.

The problem with "scan everything"

Full-body scans reliably find something — radiologists call them incidentalomas, spots and nodules that would never have caused harm. Each one buys you follow-up imaging, sometimes biopsies, months of worry, and occasionally real complications — for findings that overwhelmingly didn't matter. That's why no major guideline recommends full-body screening of healthy adults. The fear-checkup industry knows this and sells it anyway, because a scary incidental finding makes you a customer for life. We'd rather earn the opposite reputation: the physical that tells you the truth about what's worth looking for.

What the year includes

A real intake, before any testing. Your history, your family history, your medications, your prior results — reviewed by a physician first, because which tests make sense depends on who you are. The Sane Physical is a plan for you, not a fixed panel sold to everyone.
The testing that's earned its place. Built from national guidelines for your age, sex, and risk factors — the blood work, blood-pressure and metabolic assessment, and the age-appropriate cancer screenings that have real evidence behind them, coordinated with what your own doctor already ordered so nothing is duplicated.
A physician hour on the results. Not a portal message — a scheduled hour with a physician walking through every result: what it means, what trend to watch, what changes are actually worth making, and the written plan for the year.
The "what NOT to scan" review — in writing. Every tempting test you've seen advertised — full-body MRI, whole-genome panels, annual everything — addressed by name for your specific situation: recommended, not recommended, or honestly uncertain, each with the reason. When your situation genuinely warrants an advanced test, it will be on the list; when it doesn't, we'll tell you what the ad didn't.
From a real results review (details changed)
"You asked about the full-body MRI your friend got. For you — no symptoms, and the risk profile we just went through — it has roughly a coin-flip chance of finding an incidental something that leads to more scans and essentially no chance of extending your life, which is why no guideline recommends it. The unglamorous tests we ran are the ones with the mortality evidence. Here's the one number from today worth acting on…"

Common questions

Is this concierge medicine? No ongoing-care membership, no retainer for access. It's one deep, honest, physician-led checkup per year — designed to make your regular care better, not to replace it.

What if something real turns up? Then you'll hear it plainly, with the mechanism, the next step, and a written summary for your own doctor — who takes it from there. Finding real things early is the point of the tests that made the list.

Why is it so much cheaper than executive physicals? Because the expensive part of those programs is the low-value imaging — the part we deliberately don't sell. Evidence-based depth costs less than indiscriminate depth. That's not a discount; it's the philosophy.
