For attorneys, legal nurse consultants and pharmacists checking whether a documented dose was in the expected mg/kg range.
Most pediatric doses, and many adult doses of high-alert drugs, are ordered per kilogram. Tenfold errors, milligram-microgram confusion, pounds entered as kilograms and adult doses given to children are the recurring patterns in medication-error cases. Enter the drug, the patient's weight, the dose documented as given, the route, the frequency and the age. The tool computes mg/kg per dose and mg/kg/day, shows the published reference range for the drug and population from a cited public source, flags a dose outside that range or one that looks like a decimal-point error, and lists the records that settle the question. Everything is computed in your browser; only the drug name and the outcome category are logged. If the drug is not in the table, the arithmetic still runs with no reference loaded.
This is a chart-review aid, not clinical guidance. The reference ranges are starting points compiled from the public sources cited on each row. Dosing varies by indication, age band, renal and hepatic function, prior exposure, institutional protocol and the specific product. A dose outside the range here is a question for a pharmacist or physician expert, not a conclusion. A dose inside the range does not mean it was correct for this patient.

How to use the result
Check the weight first. Most apparent overdoses in children trace to the weight: pounds charted as kilograms, a stale weight from a prior visit, or an estimated weight never replaced. The tool shows what the mg/kg would have been if the units were swapped; if that number lands in range, the weight record is the issue to chase.
Match the indication. Many drugs here have more than one labeled range (for example meningitis vs other infections, status epilepticus vs maintenance, anaphylaxis vs cardiac arrest). The row used is listed with its indication; read the other rows for the drug before treating a dose as out of range.
Treat the flag as a question for the expert. A pharmacist or physician decides whether a deviation was clinically significant, intentional (level-guided, renal adjustment, tolerance) or an error. This page reports arithmetic against a published range and nothing more.
Look at the whole chain. An out-of-range dose that was ordered, verified by pharmacy, dispensed and administered passed several checks. The records below show which check failed and who documented what.
Verify the range against the cited source and the product actually used before it goes into a memo or expert packet. Labels and guidelines are revised.
What to request next
The original order (CPOE record or paper order) with the ordering provider, time, dose, units, route, frequency and the weight the system used for any dose calculation
The weight documentation at the time of the order: admission weight, the scale or stated/estimated source, units, who entered it and when it was last updated
Pharmacy verification log: verifying pharmacist, time, any dose-range alert that fired, the override reason, and the pharmacy's weight and dosing-weight fields
MAR / eMAR entries for every administration, with the barcode scan record, the dose actually documented and the nurse's independent double-check if the policy requires one
Smart-pump or infusion-pump logs (programmed concentration, rate, dose and any library alerts or overrides) for infusions
Automated dispensing cabinet transactions and compounding records for the product and concentration actually used
The dose-range checking configuration and alert history in the EHR and pharmacy system for this drug
Facility policies on weight-based dosing, pediatric dosing limits, high-alert medications and independent double checks in force on the date
Incident report, pharmacy medication-error report and any root-cause analysis for the event, where discoverable
