Medical Record Review Time and Fee Estimator

Quoting a record review is guesswork until you have done a hundred of them. This calculator turns the page count and record mix into hours by task (intake and organization, reading and extraction, drafting, quality check), shows the pages-per-hour assumption behind every number so you can argue with it, and produces a fee range and a quote paragraph you can paste into an engagement email. Built for legal nurse consultants and attorneys who bill or budget record review by the hour. Nothing you enter leaves your browser except a one-time anonymous usage count.

1. Pages by record type

Converted at 15 pages per MB (our assumption for scanned medical-record PDFs; native-text PDFs run far more pages per MB). Added to the inpatient line if the type fields are empty.

2. Work product and rate

Typical published range for independent legal nurse consultant record review is roughly $100 to $200 per hour, with testifying experts and physician reviewers higher. Source: trade materials such as AALNC educational resources and LNC rate guides, not a peer-reviewed survey; confirm current figures for your market.

The AI-assisted pages-per-hour figures are MedLegal AI's own assumptions, not measured in a published study. They are shown next to the manual figures so you can judge them.

Assumptions behind the numbers

Honest note on sources. We did not find a peer-reviewed study that measures medical-record review speed by record type. The manual pages-per-hour figures are MedLegal AI's working assumptions, kept inside the 50 to 100 pages-per-hour rule of thumb that circulates in legal nurse consultant training material for careful chronology work, with billing and report pages faster and dense inpatient and long-term-care records slower. The AI-assisted figures are the site's own assumptions about verification-reading speed when software produces the first draft; they are not independent measurements. Treat both as a starting point and replace them with your own tracked time as soon as you have it.

    How to use the result

    1. Quote the range, not the midpoint. Record productions grow after the first set arrives, and duplicates, blank pages and fax cover sheets shrink the real page count. A range with a stated page assumption survives both.
    2. Say what is included. The hours assume one pass of reading, one draft and one quality check. Re-review after a supplemental production, a meeting with the attorney, or a second format of the same work product are extra and should be said so in the engagement letter.
    3. Adjust the pages-per-hour figures to your own history. If you have tracked time on prior cases, replace the assumptions with your averages. The model is linear in page count, so changing one figure moves one line in the table.
    4. If you use AI assistance, bill the verification honestly. The AI-assisted column still charges for reading every entry against the source. A chronology that was not verified line by line should not be sold as a verified chronology.
    5. Keep the assumptions with the quote. Paste the quote paragraph and the assumption list into the engagement email so the client sees how the number was built.
